  Municipal enterprises, doctor-FOPs, private clinics — everyone practicing medicine; an NHSU contract doesn't replace the license.

• Material base

  A standards-compliant office, documented equipment, sterilization — checked at issuance.

• Staff

  Specialist certificates, experience, appointment orders for responsible persons — all in the application.

• Renewal

  The license is perpetual, but changes of address/services/staff require re-issuance within a month.

Practicing without a license means fines up to 5% of annual revenue + activity ban. A doctor-FOP's license is tied to the office address — moved? Re-file.
