CLAIM

1. I am employed by your company in the position stated above.

2. As of the date of this claim, the company owes me wages in the amount of __________ UAH for the period: __________.

3. Under Article 115 of the Labor Code of Ukraine, wages must be paid regularly at least twice a month. Delayed payment violates labor legislation.

4. Under Article 117 of the Labor Code of Ukraine, in case of delayed payment the employer must compensate the employee for the loss of purchasing power (indexation).

I DEMAND:

1. Pay the wage arrears in the amount of __________ UAH within 10 calendar days from receipt of this claim.

2. Provide a written response.

If this claim is not satisfied, I will be forced to apply to the State Labor Service and/or the court to protect my rights, which will entail additional costs for the company.
