Medical documents abroad

Medical documents for relocation: translation, insurance, vaccination, prescriptions

What to translate

Discharge summary/medical history, lab results, vaccination card (form 063), chronic-condition and medication certificates. Sworn/certified translator only.

Vaccinations

Vaccination card in international format; MMR, DTP, polio are mandatory for EU schools. Check titres: without a card, serology replaces proof of vaccination.

Insurance

Until the local policy starts — travel/relocation insurance. Pre-existing conditions usually aren't covered for the first 6–12 months — keep a reserve or extend Ukrainian coverage.

Prescriptions & medicines

Drug list by INN (international name, not brand), dosages, translated prescriptions. Narcotic/psychotropics need a separate certificate and border declaration.

Chronic conditions

2–3 months of medication + English discharge summary → register with a local doctor in the first weeks. GP queues in the EU take weeks, not days.

Apostille/legalization

Medical documents for official procedures (disability, driving, benefits) may need an apostille — check the receiving body's requirement.

Where in Ukraine

Sworn translators (Justice Ministry register), MoH vaccination-translation forms, notarized copies. E-translations are rarely accepted.

Medical certificates must be translated by a sworn translator of the destination country — insurers and doctors reject ordinary translations. Ukrainian prescriptions don't work abroad: the prescription must be re-issued locally, which is what the translated discharge summary is for.
