Choose a sanatorium from the registry via the PFU portal/Diia — queue forms automatically.
Eligibility:
Disabled, combat veterans, workplace injury victims, specific diagnoses — list at the PFU.
Rehab centers:
After injuries/strokes/surgeries — by doctor's referral; military — dedicated MOD/MOH centers.
Coverage:
Accommodation, meals, basic procedures; travel and some procedures usually out-of-pocket.
Documents:
Passport, tax ID, doctor's certificate (form 070/o), benefit category certificate.
Queue:
Fewer vouchers than applicants — apply early in the year; refusal must be written.
Alternative:
Outpatient rehab under the NHSU package in state hospitals — free by referral.
