__________________ (EDRPOU: __________), represented by __________________, hereinafter referred to as the "Customer", on the one part, and

__________________ (RNOKPP: __________), residing at __________________, hereinafter referred to as the "Contractor", on the other part,

have concluded this Agreement as follows:

1. SUBJECT

1.1. The Contractor undertakes to render the following services: __________________, and the Customer undertakes to accept and pay for them.

1.2. This Agreement is a civil-law contract and does not establish employment relations between the parties.

2. PRICE AND PAYMENT

2.1. The price under this Agreement is __________ UAH.

2.2. Payment terms: within 5 banking days after signing the certificate of services rendered / works completed.

2.3. Where the Customer is a tax agent, PIT (18%) and military levy (5%) shall be withheld from the Contractor's remuneration in accordance with the Tax Code of Ukraine.

3. TERM

3.1. The services/works shall be rendered/performed within: __________.

3.2. Acceptance is confirmed by a certificate of services rendered / works completed signed by both parties.

4. LIABILITY

4.1. For delay in performance the Contractor shall pay a penalty of 0.1% of the price for each day of delay, but not more than 10% of the price.

4.2. The Customer bears no liability for the Contractor's social insurance; the Contractor pays SSC (ESV) independently where applicable.

5. FINAL PROVISIONS

5.1. The Contractor performs the obligations personally unless otherwise agreed in writing.

5.2. This Agreement is drawn up in two copies having equal legal force.

5.3. Disputes are resolved under the legislation of Ukraine.
