                               STATEMENT OF CLAIM

The child lives with me. The defendant does not participate in the upbringing and maintenance of the child and does not provide funds voluntarily.

According to Articles 180 and 181 of the Family Code of Ukraine, parents must support their child until adulthood.

Based on the above, I REQUEST:
1. To collect from the defendant [Full name] in favor of [Full name] child support for the maintenance of the child [Child full name] in the amount of 1/4 of all types of the defendant's income monthly, starting from the date of filing this claim until the child reaches adulthood.
